As a guest of the Palace Hotel and with my signature below, I do hereby agree to the following hotel policies regarding
having my pet on-premises during my stay.
 
1 I understand that the nightly fee for having my pet in my hotel room is $25.00 + tax per pet (2 pet max) in addition to the hotel's nightly rate.  All in-room pet's must be registered with the Front Desk upon check-in.  Failure to register my pet may result in my eviction without refund.
 
2 I understand and authorize that my credit card be charged for any hotel damages that result or are caused by my pet during my stay (see #8 below).  In lieu of a credit card authorization, I will provide hotel with a $200.00 cash deposit which will be returned to me upon check-out after my room has been inspected and cleared by hotel personnel.
3 I understand my pet is not permitted in any public/common areas of the hotel (Lobby, Breakfast Room, etc.) at anytime during my stay.  As a guest with a registered pet, I agree to hold the Palace Hotel harmless from pet injury, accidents or death as well as the overall safety of my pet while on-site to include hotel balconies, parking areas, etc.
 
4 I understand that I will leash or crate my pet, if applicable, at all times while on-premises (except when in my assigned room); when "coming in or out" of the hotel and accept all legal liability for damages my pet may cause to any guest or to the contents of my hotel room or hotel property; and understand my pet shall at all times remain under my sole care, custody and control and further agree that Hotel personnel are not authorized or permitted to supervise, feed or "pet sit" any registered animal during my stay.  Pets that create a noisy or disturbing influence on the hotel's normal operations or disrupt the quiet use of this facility for other guests, may result in my eviction without refund.
5 I understand that I will be issued a "Dog In Room" sign to display on the outside of my room door to alert hotel personnel of my pet "in residence" and that I will pay a $5.00 fee for this sign if not returned to the Front Desk upon check-out.
6 I understand that hotel Housekeeping personnel will not enter my room for daily cleaning purposes unless my pet is "crated" or when I am present if no crate protection is provided by me.  I further understand that it is my responsibility to contact the hotel's Housekeeping Department for the purpose of cleaning my room on a daily basis before Housekeeping personnel leave the property at end of shift (2:00 pm daily).  For Housekeeping services dial 7521 or 0.
7 I understand that the hotel has assigned a specific grass area behind the hotel to walk my pet and in addressing my pet's bathroom needs.  I also understand that I must clean-up my pet's waste and dispose of same in the hotel's trash receptacle in the Palace Parking Garage.
 
8 HOTEL DAMAGE FEES:
 
I understand and agree to be held personally liable for any and all damages caused by my pet to hotel property or room contents and accept the responsibility of all fees that may be charged against my account if applicable:
In-Room Carpet Shampooing $85.00 Window Treatment Damages:  Full Replacement Cost

Pet Waste Clean-up Fee:  $10 per occurrence
